- What does my plan cover?

- How much does my plan cost?

- When do my benefits start?
- All newly eligible benefits will begin on the first of the month following 30 days. Please refer to the following table for more details: One Stop – FHCSD Wellness (gobenefits.net)
- How do I get my insurance card?
- Once enrolled, you should be receiving your medical cards from the appropriate carrier. To request new cards, you will want to each out to your carrier.

- Please note that they do not automatically issue cards to participants for the Cigna Dental program so you will want to request/access your card on their website.
- Our vision plan does not automatically issue cards. You will want to navigate to Vision Service Plan website and register your account at www.vsp.com or reach out to their customer service line at (800)877-7195 (Group number 30028835) to access your vision card and coverage information.
- Once enrolled, you should be receiving your medical cards from the appropriate carrier. To request new cards, you will want to each out to your carrier.
- How do I cancel my benefits?
- Due to IRS rules, you cannot change your elections until the next annual Open Enrollment period, unless you have a qualifying life event during the year. For more information visit: Changing Your Benefits – FHCSD Wellness (gobenefits.net)
- Can I change my plan at any time?
- You must experience a qualifying event in order to make any changes to your benefits outside of the Open Enrollment period.
- Who Can I add to my plan?
- Eligible family members include:
- Your legally married spouse
- Your registered domestic partner (RDP) and/or his/her children, where applicable by state law
- Your children who are your natural children, stepchildren, adopted children or children for whom you have legal custody (age restrictions may apply). Disabled children age 26 or older who meet certain criteria may continue on your health coverage.

- How do I reach our EAP?
- Call 800-344-4222 24/7 or visit employees.concernhealth.com and log in with your company code FHCSD for easy access to all available Concern services.
Flexible Spending Accounts
- How do the flexible spending accounts (FSAs) work?
- The Igoe FSAs are one way to cut down on your expenses—and we offer two. With the FSA, you set aside aside a portion of your pre-tax income. Because that portion of your income is not taxed, you pay less in federal income, Social Security and Medicare taxes. You can then use those funds to pay for qualified health care and/or dependent care expenses.
- Do I lose all my funds if I do not use them by the end of the plan year?
- Health care FSA: Unused funds of up to $640 from one year can carry over to the following year. Carryover funds will not count against or offset the amount that you can contribute annually. Unused funds over $640 will NOT be returned to you or carried over to the following year.
- Dependent care FSA: You can incur expenses through December 31, 2024. Unused funds will NOT be returned to you or carried over to the following year.
- How much can I contribute for the plan year?
- Healthcare FSA: For 2024, you may contribute up to $3,200 to cover eligible health care expenses incurred by you, your spouse and your children up to age 26.
- Dependent Care FSA: For 2024, you may contribute up to $5,000 (per family) to cover eligible dependent care expenses ($2,500 if you and your spouse file separate tax returns).
Life and AD&D Insurance
- How much does it cost?
- This benefit is provided at no cost to you and is fully funded by FHCSD.
- What does it cover?
- Term life with accidental death & dismemberment (AD&D) insurance can provide money for your family if you die or are diagnosed with a terminal illness. For more information, click here for the plan summary.
- How do I assign beneficiaries?
- You will be able to assign beneficiaries within Workday within your New Hire and Open Enrollment events. You are able to make changes any time by submitting a “Change Beneficiary” event in Workday.
403(b) Retirement
- How do I enroll in the 403(b) retirement plan?
- If you are eligible, you are auto enrolled at a 6% contribution rate and are eligible to begin contributing the 1st of the month following hire date.
- Can I change my contribution amount?
- Please note, you may change your contribution anywhere from 0-100%. To do so you will need to navigate to Principal’s site at www.principal.com and register your account. You may reach out to Principal directly at 800.547.7754 if you have any questions regarding your account or have difficulties logging in.
- How much does FHCSD match?
- FHCSD will match 50% of your contribution on the first 10% of your pay.
403(b) Retirement
